All school districts now require that parents complete a transportation request form (ACT372) for each of their students in grades Kindergarten through Eighth every school year. Each school district has a specific form or process. Please click on your district below.

Please complete the request for your school district and send them to the district, not St. Elizabeth, as soon as possible, and before July, 1, 2021 or they will not guarantee transportation in September.
