All school districts require that parents complete a transportation request form (ACT372) for each of their students in grades Kindergarten through Eighth every school year. Each school district has a specific form or process. Contact the transportation department within your school district.

Forms must be submitted no later than June 30th of every school year in order to receive busing for the first week of school. If your busing needs change or your address information changes, please fill out another form and submit it to to your district ASAP.
